Terms and Conditions
Purpose |
Purpose of the Scholarship: Annuvia, its officers, directors and employees and agents notifies all interested college students of the Annuvia Public Safety Scholarship! The Annuvia Public Safety Scholarship is designed to teach, encourage and promote public health and safety or emergency medical services and make the world a safer place to live and work. The amount of the scholarship is $500.00 and it is awarded in the fall, once per year (specific date will vary year over year). |
Agreement

Agreement to Application Requirements: By applying for this scholarship, you agree to the application requirements set forth herein.
Eligibility: The scholarship is open to legal residents of the United States of America, who are 18 years of age or older as of the date of enrollment at his or her educational institution, who are not employees or direct relatives of employees of Annuvia and who are attending a college or other post-secondary educational institution on a full-time basis. |
Deadline |
Application Deadline: You must complete your application no later than July 31 of each year to be entered for consideration. |

Application Process:
a. Review: You must review these terms and conditions before applying. You’re also encouraged to review more about Annuvia’s history, articles, and our products and services, so that your essay relates, in some way, to our industry focus. b. Essay: You must submit an original 500+ word essay. Write an essay about how you propose the emergency medical services industry improve public safety, emergency response, or general public safety. Your essay should incorporate facts and details and, where possible, accurate industry research and information. Your essay may pertain to medical practices, emergency communication, or a new system, device, or method of caring for patients. Essays must be original and written by the applicant. We will check essays for plagiarism, re-writes, and duplication. c. Submission: You must submit your essay with your registration information to: Scholarships d. Review: Each Fall, we review the essays for content, organization, grammar, spelling and creativity. our review panel will make a determination as to the winner based on the goals set forth herein. |
Selection Process |
Selection Process:The review panel will review all entries for originality of the submitted essays while ensuring that each submission stays within the guidelines set forth herein. |
Notification |
Notification: By September 30, of each year, Annuvia will notify the applicant with the highest rated application and send out an Acceptance Form to that applicant. The Acceptance Form will contain certification of eligibility (including proof that the applicant will be 18 years of age or older as of his or her date of enrollment at the educational institution and proof of full time student status with active enrollment documentation), an address at which they may receive certified, insured, and return receipt restricted U.S. mail, liability releases, and publicity releases granting Annuvia the right to use and publish their name, biographical information, statements made regarding Annuvia, or the promotion, portrait, picture, voice and likeness of the winning applicant for trade, advertising and promotional purposes, without additional consideration, notification, review, and/or approval, worldwide, and/or on the internet. If, within 14 days, Annuvia does not receive the completed Acceptance Form from the recipient, then the offer of scholarship will be revoked and a “runner up” submission will be selected. Annuvia reserves the right to award the scholarship to the applicant who is the author of the next best essay. |
Awarding |
Awarding the Scholarship: Upon Annuvia timely receiving a completed Acceptance Form, it will remit payment directly to the college or other post-secondary educational institution set forth in the winning applicant’s application. |
Additional Details |
Disqualification: At its sole discretion, Annuvia reserves the right to disqualify applicants who we believe or has reason to believe is violating the terms and conditions of this scholarship application.
Taxes: Any and all applicants are solely responsible for any and all applicable state and/or federal taxes, and/or any and all other fees associated with this scholarship. As is: Everything in this scholarship application is provided as is, without warranty of any kind, express or implied. Cancellation: At its sole discretion, Annuvia reserves the right to cancel this scholarship at any time without penalty. Release, Indemnification, and Hold Harmless: By applying for this scholarship, applicants agree to release, indemnify, and hold harmless Annuvia, its officers, directors and employees and agents from any and all liability for any and all claims that may arise from applying for this scholarship. Jury Trial Waiver: Applicants waive any and all rights they may have to any and all trials by jury. Law, Jurisdiction, and Venue: Each applicant agrees that any dispute arising out of this scholarship program or application process shall be governed by the internal laws of the State of California, without regard to its conflicts of laws provisions. Further, each applicant irrevocably submits to the personal jurisdiction and venue of the San Francisco County in the State of California and waives any objection thereto. Integration Clause: This scholarship application constitutes the complete and final agreement, supersedes all applications, is the full and exclusive statement of the official scholarship application, and may only be modified in writing by Annuvia. |
